Abstract

The United Arab Emirates (UAE) has experienced rapid economic growth that was paralleled by a drastic rise in non-communicable diseases (NCDs); primarily cardiovascular diseases (CVDs), which account for 40% of mortality in UAE. CVDs have complex etiology, interplay, and accumulation of many risk factors. Understanding the clustering and interrelationships between common risk factors like obesity, deglycation, dyslipidemia, hypertension and central obesity, and the associations with social and lifestyle determinants are warranted. The study aimed to estimate the burden of cardiometabolic risk factors (CRFs), their interrelationship, and their associations with other social and lifestyle determinants in a young Emirati population. Data were drawn from the UAE Healthy Future Study collected between 2016 and 2018. The information was collected through questionnaires, physical measurements, and blood samples. Age-adjusted and gender-specific prevalence of CRFs were estimated, and appropriate regression models were used to determine the interrelationships and associations of the CRFs. A total of 5,167 eligible participants aged 18-40 were included in the analysis. The age-adjusted prevalence rates were 26.5% for obesity, 11.7% for deglycation, 62.7% for dyslipidemia, 22.4% for hypertension and 22.5% for central obesity. Dyslipidemia had the highest comorbidity rate, up to 80%, with other CRFs, followed by obesity. Obesity had the strongest interrelationship with other CRFs. Education, employment, smoking and family history of NCDs had significant associations with some CRFs. Forty per cent of the population had ≥2 CRFs, and the accumulation was higher in men than women; 47.8% vs 28.1%, respectively. The burden of CRFs was affected by age and social factors and was significantly different across BMI classes. CRFs and their clustering are highly prevalent in young adults, including those with normal BMI. This should be considered in the design and targeting of group-specific measures for CVDs and other NCDs prevention. Further research is needed to investigate how the clustering manifests in young adults to prevent the early rise of NCDs in the UAE.

شھدت دولة الإمارات نموا سریعا في الاقتصاد، وواكب ھذا النمو ارتفاع حاد في الأمراض غیر المعدیة؛ حیث تعد أمراض القلب والأوعیة الدمویة في المقام الأول، والتي تمثل 40% من أسباب الوفیات في دولة الإمارات العربیة المتحدة. وعلاوة على ذلك، فإن ھذه الأمراض القلبیة الوعائیة لھا مسببات متعددة العوامل ومعقدة. لذلك یتوجب علینا فھم التكتل والعلاقات المتبادلة بین عوامل الخطر المنتشرة مثل السمنة وسكر الدم وارتفاع نسبة الدھون فالدم وارتفاع ضغط الدم والسمنة المركزیة، ومن المھم فھم ارتباطھا بالعوامل الاجتماعیة الأخرى ومحددات نمط الحیاة. ھدفت ھذه الدراسة إلى تقدیر عبء عوامل الخطر القلبیة والتحقیق في ترابطھا مع بعضھا البعض، وارتباطھا بالعوامل الاجتماعیة ومحددات نمط الحیاة الأخرى في الإماراتیین الشباب. تم استخلاص البیانات من دراسة "مستقبل صحي للإمارات" التي تم جمعھا بین عامي 2016 و 2018 ، حیث تم جمع المعلومات من خلال استبیانات وقیاسات بدنیة وعینات الدم. قدُِرّ عبء عوامل الخطر لكل فرد، وقُدِّر معدل انتشار ھذه المعدلات حسب العمر والجنس، واستخُدمت نماذج الانحدار اللازمة لتحدید العلاقات المتبادلة بین ھذه العوامل وارتباطاتھا بالعوامل الأخرى شمل التحلیل 5,126 مشاركا من 40-18 سنة وكانت معدلات الانتشار المعدلة حسب العمر 26.5 % للسمنة، و 11.7 % لسكر الدم، 62.7 % لارتفاع الدھون فالدم، و 22.4 % لارتفاع ضغط الدم 22.5 % للسمنة المركزیة. كان ارتفاع نسبة الدھون فالدم أكثر عامل خطر تواجد مع غیره من العوامل لحد 80% من الوقت، ویتلوه السمنة المفرطة. وكان للسمنة أقوى علاقة مترابطة عوامل الخطر الأخرى. أثبتنا أن لأحوال التعلیم والعمل والتدخین والتاریخ الأسري للأمراض غیر المعدیة لھ روابط كبیرة مع بعض عوامل الخطر المذكورة. ووجدنا أن 40% من شریحة الشباب یراكمون أكثر من عاملي خطر في آن واحد، وكانت نسبة التراكم أعلى بین الذكور على الإناث 47.8 % مقابل 28.1% . تأثر التراكم بشكل كبیر بالعمر والعوامل الاجتماعیة. وكان عبء التراكم لعوامل الخطر مختلفا بشكل كبیر عبر فئات مؤشر كتلة الجسم. إن ارتفاع عوامل الخطر للأمراض القلبیة وتراكمھا أصبح خطرا یھدد بشكل كبیر فئة الشباب في دولة الإمارات العربیة المتحدة حتى من ھم دون ارتفاع الوزن المفرط. ویتأثر عبء وتجمیع عوامل الخطر ھذه بالعمر ونوع الجنس والتعلیم والعمالة والتدخین والتاریخ الأسري للأمراض غیر المعدیة. وینبغي أن یؤخذ ذلك في عین الاعتبار عند تصمیم واستھداف التدابیر الخاصة بكل مجموعة للوقایة من الأمراض القلبیة الوعائیة بصفة خاصة وغیرھا من الأمراض غیر المعدیة بوجھ عام. وثمة حاجة إلى إجراء مزید من البحوث للتحقیق في كیفیة ظھورھا بین الشباب لمنع الزیادة المبكرة للأمراض غیر المعدیة في الإمارات العربیة المتحدة.
