Abstract

After more than a century of investigation, it has become evident that cancer development and progression occur at the expense of a dysfunctional immune system. This realization has ushered cancer immunotherapy as a new modality of treatment. In the last decade, the use of immune checkpoint inhibitors to treat cancer patients resulted in unprecedented and durable clinical benefits. However, the response rate in many cancers remains rather modest, with colorectal cancer being at the lower end of the spectrum. Previous work from our laboratory demonstrated the efficacy of using attenuated bacteria as immunomodulatory anti-cancer agents. The current study investigated the potential of utilizing a low dose of attenuated Salmonella enterica serovar Typhimurium (henceforth S. typhimurium) to enhance the therapeutic outcome of PD-L1 blockade, an immune checkpoint inhibitor, in a preclinical model of colon cancer. In our study, the response of MC38, a murine colon adenocarcinoma, to a suboptimal dose of anti-PD-L1 monoclonal antibody (aPD-L1 mAb; 5 mg/kg/dose) was quite variable, with only ~30% of the mice responding to the treatment. The Salmonella strain used in this study is a well-characterized, double auxotrophic mutant known as BRD509, with an LD50 of >5x106 CFUs when administered systemically. Utilizing multi-color flow cytometry and immunohistochemistry, we demonstrated that even when used at a very low dose (5x103/mouse), BRD509 was able to modulate the tumor microenvironment through enhancing the infiltration of CD4+ T cells, increasing the antigen presentation potential of myeloid cells, and decreasing the percentage of tumor-infiltrating lymphocytes that express inhibitory checkpoint ligands PD-1 and LAG-3. Combination treatment with BRD509 and αPD-L1 mAb increased the response rate to 100% and led to ~80% inhibition in tumor growth compared to controls. Mechanistically, the enhanced response correlated with considerable alterations within the tumor microenvironment including (1) an increase in tumor infiltration by CD45+ immune cells including CD4+ and CD8+ T cells, (2) a decrease in the percentage of intratumoral Ly6G+ granulocytic myeloid cells and (3) an increase in the percentage Ly6G- Ly6Chi macrophages that express MHC class II proteins. These changes were accompanied by a significant increase in apoptotic cells within the tumor parenchyma. Gene expression analysis by real-time PCR of tumor tissues revealed changes in the expression of various chemokines and inflammatory cytokines that were consistent with the observed alterations in the cellular constituents of tumors treated with BRD509 and αPD-L1. Taken altogether, the current study demonstrates that a novel combination treatment utilizing attenuated Salmonella and PD-L1 blockade could significantly improve the outcome of immunotherapy for colorectal cancer.

بعد أكثر من قرن من الأبحاث، أصبح من الواضح أن نمو وتطور السرطان يحدثان بسبب اختلال في عمل الجهاز المناعي. وقد أدى هذا المفهوم الجديد إلى جعل العلاج المناعي وسيلة جديدة لعلاج السرطان. في العقد الأخير، أدى استخدام مثبطات نقاط التفتيش المناعية لعلاج مرضى السرطان إلى نتائج سريرية إيجابية غير مسبوقة. ومع ذلك، فإنّ معدل الاستجابة للعديد من أنواع السرطان لهذا العلاج الجديد لا يزال متواضعاً إلى حد ما مع وجود سرطان القولون في الطرف الأدنى من الطيف. وفي عملٍ سابق، تم إثبات فعالية استخدام البكتيريا المضعّفة كعوامل مضادة للسرطان. في هذه الدراسة، تم التحقق من إمكانية استخدام جرعة منخفضة من السالمونيلا المعوية المضعّفة (Salmonella enterica serovar Typhimurium) لتعزيز الفعالية العلاجية لمثبط PD-L1 -مثبط نقطة تفتيش مناعية- في تجربة ما قبل السريرية باستخدام النموذج الحيواني لسرطان القولون (MC38). في هذا النموذج، كان معدل استجابة الفئران الحاملة للورم إلى جرعة منخفضة من الجسم المضاد PD-L1 أحادي النسيل (5 ملجم/كغم/جرعة) ضعيف ويصل إلى ما يقارب 30% فقط. ومن الجدير بالذكر أن سلالة السالمونيلا المستخدمة في هذه الدراسة هي متحولة ثنائية عونية التّغذّي (double auxotrophic mutant) معروفة باسم BRD509 وتتسم بمتوسط جرعة قاتلة (LD50) يزيد عن 5x106 وحدة (CFUs) عند إعطائها جهازياً. في هذا البحث، قمنا باستخدام قياس التدفق الخلوي المتعدد الألوان (multi-color flow cytometry) والتصبيغ النسيجي الكيميائي (immunohistochemistry) لدراسة التغييرات الخلوية التي أحدثها العلاج في الورم. أظهرت النتائج أنّه حتى عندما يتم استخدام BRD509 بجرعة منخفضة جداً (5x103/فأر)، فإنها تعدل البيئة الدقيقة للورم من خلال تعزيز قدوم خلايا CD4+T، زيادة إمكانية الخلايا النخاعية (myeloid cells) لعرض مولدات الضد، وتقليل النسبة المئوية للخلايا اللمفاوية في الورم التي تظهر نقاط التفتيش المناعية المثبطة، PD-1 وLAG-3. في المقابل أدى العلاج المركب من الجسم المضاد PD-L1 أحادي النسيل و BRD509 إلى زيادة معدل استجابة الفئران إلى 100%. كما أدى إلى تثبيط نمو الورم بما يقارب 80% مقارنة بالفئران التي لم تتلق العلاج. وعند البحث عن آلية عمل العلاج المركب، تبيّن أن الاستجابة المعززة ارتبطت مع تغيّرات في البيئة الدقيقة للورم ملخصة في (1) زيادة في تدفق الخلايا المناعية CD45+ إلى الورم بما في ذلك خلايا CD4+T وخلايا CD8+T، (2) انخفاض في النسبة المئوية للخلايا النخاعية المحببة (Ly6G+) في الورم، (3) زيادة في النسبة المئوية للبلاعم (LY6G- LY6Chi) التي تظهر بروتينات MHC الفئة الثانية. وكانت هذه التغييرات مصحوبة بزيادة كبيرة في الخلايا التي خضعت للموت المبرمج داخل الورم. كما كشف تحليل التعبير الجيني بواسطة تفاعل البلمرة المتسلسل بالزمن الحقيقي (RT-PCR) في أنسجة الورم عن تغييرات في التعبير عن مختلف الكيموكينات والسيتوكينات الالتهابية والتي كانت متوافقة مع التغييرات الملحوظة في المكونات الخلوية للأورام المعالجة بمزيج من الجسم المضاد PD-L1 أحادي النسيل مضاد و BRD509. تظهر الدراسة الحالية، في مجملها، على أن العلاج المركب الجديد باستخدام السالمونيلا المضعّفة ومثبط PD-L1 يمكن أن يحسن بشكل كبير نتيجة العلاج المناعي في سرطان القولون والمستقيم.
