Abstract

Pharmacogenomics (PGx) enables the personalisation of drug therapy based on genetic variation, with the potential to improve safety, efficacy, and cost-effectiveness. Cardiovascular care is a priority area for PGx due to well-established gene–drug interactions, yet successful integration requires readiness across patients, clinicians, and payers. This thesis aimed to assess stakeholders' knowledge, perceptions, and system-level requirements for PGx implementation in cardiovascular practice in the United Arab Emirates (UAE), and to generate actionable, context-specific recommendations for national adoption.

Three interlinked studies were conducted. A quantitative survey of 425 cardiovascular patients assessed their knowledge, perceptions, preferences, and willingness to undergo and pay for PGx testing. Analyses included descriptive statistics, exploratory factor analysis, cluster analysis, and multivariable logistic regression. A qualitative study of 15 cardiologists used an abductive design, incorporating semi-structured interviews and the Consolidated Framework for Implementation Research (CFIR) to investigate readiness, perceived clinical utility, and operational barriers. The qualitative study of 12 insurance decision-makers and policy stakeholders also employed an abductive design, analysed using the Technology Acceptance Model (TAM), Institutional Theory, and SWOT analysis to explore reimbursement models, cost-effectiveness considerations, and regulatory alignment. Ethics approval and informed consent were obtained for all studies.

Among patients, 50.8% had prior PGx awareness, 55.3% demonstrated good knowledge, and a high willingness to test was observed when a physician recommended it. Awareness was significantly associated with female gender, genetic disease history, hospitalisation for medication side effects, and frequent medication use (p < 0.05). Cluster analysis revealed three distinct readiness profiles. Cost concerns were common, with 34.8% of participants unwilling to pay despite the absence of clear pricing information. Safety was the top priority for 60.2% of respondents, while 35.3% preferred preemptive testing. Cardiologists valued PGx, especially for clopidogrel, warfarin, and statins, but cited low training, a lack of UAE-specific guidelines, limited local testing capacity, slow turnaround times, and no reimbursement as major barriers. Opportunities included piloting PGx in chronic disease clinics, integrating electronic health, and implementing multidisciplinary care models. Insurers expressed conditional support, contingent on UAE-specific cost-effectiveness evidence, governance clarity, and policy alignment. Executives prioritised strategic fit and ROI; middle managers highlighted operational barriers, including high costs, reliance on international labs, and fragmented policies. Opportunities included phased rollouts, public–private partnerships, and investment in local lab capacity.

Integrating PGx into UAE cardiovascular care is achievable but demands coordinated action across policy, practice, and infrastructure. While patients, clinicians, and insurers recognise its potential to improve outcomes and reduce adverse drug reactions, readiness is constrained by awareness gaps, limited training, poor communication of existing guidelines, infrastructural challenges, and unclear reimbursement pathways. Addressing these gaps through targeted education, workforce development, strong local evidence, and policy reform will be essential. By taking a phased, evidence-based approach, the UAE could establish itself as a regional leader in personalised medicine and offer a scalable model for other health systems seeking similar innovations.
